A Declaration to be obtained by employers from employees, out sourcing staff, contractors and sub-contractors concerning the conflict of interest, if any, held by them in discharging their duties and their interests in the persons, firms, concerns, or companies or other entities that act as service providers or suppliers of goods to the employer. The declaration also declares that none of the family members, relatives or personal friends who have interest in the supplier of services or goods to the employer.  The declaration is to declare the existing conflict of interest if any and an undertaking to declare any changes in the present status or a conflict of interest that may arise in future.
